Your Responsibilities
Conduct patient assessments and develop personalized Plans of Care (NO OASIS!)
Monitor and document changes in patients’ conditions and needs
Communicate proactively with patients and their families regarding care plans and health updates
Qualifications
Current, unrestricted PT license in Texas
Master’s degree in Physical Therapy (Required)
CPR Certification & Negative TB Test (or Chest X-ray)
Minimum one (1) year of experience in a home health agency, primary care clinic, or healthcare facility (preferred)
Strong assessment, analytical, and critical-thinking skills
Valid driver’s license & reliable transportation (must be willing to travel up to 30 miles)
Ability to stand, walk, squat, bend, kneel, reach, twist, push, and pull, as needed (must be able to lift up to 50 lbs)
