preaa PSA: It’s important to take a second to read the vial of drug you grab and confirm it’s what you’re wanting to use. At first glance, Ephedrine and Epinephrine look very similar. Both are concentrated solutions, and if used incorrectly, can cause serious harm. 
Ephedrine works by indirect stimulation of the adrenergic receptor system by increasing the activity of norepinephrine at the postsynaptic α and β receptors ultimately causing vasoconstriction.

Epinephrine acts on alpha and beta-adrenergic receptors and is the strongest alpha receptor activator.  Important effects of epinephrine include increased heart rate, myocardial contractility, and renin release via beta-1 receptors. Beta-2 effects produce bronchodilation which may be useful as an adjunct treatment of asthma exacerbations as well as vasodilation, and tocolysis. ⁣⁣⁣

preaa Frequently I get asked what I did before becoming an Anesthesiologist Assistant Student. I attended The University of Georgia (GO DAWGS) for my undergraduate Bachelors degree with Pre-Medical Intent. After taking my MCAT, applying to medical schools (MD programs), and graduating top 7%... I became a flight attendant for a major US airline and traveled around the world for free! What a change, right? 
Since the Anesthesiologist Assistant profession is fairly new (only 50 years old this year!), a lot of students in the program come from different backgrounds due to learning about the CAA profession late in college or shortly after (like me!). Just like applying to MD medical school programs, you can major in anything you’d like, as long a you fulfill the mandatory requirements. This makes each of our individual skill sets unique, but requiring similar background academics wise. Don’t get me wrong, a lot of our students are fresh out of undergrad- hitting the ground running with the Anesthesiologist Assistant profession! Man, I just wish I would’ve known about the AA profession sooner! 
If you’re learning about this career later in life, don’t let your nontraditional background deter you! 
So what educational background do you have to have in order to apply to the AA program? 
#1. Bachelors degree from an accredited college or university 
#2. Prerequisite courses: Biochemistry, Biology, Human Anatomy, Human Physiology, Chemistry, Organic Chemistry, Physics, Calculus, Statistics, and English

Contraindications to neuraxial anesthesia include patient refusal, infection, bleeding diathesis, and ICP (?). Bacteremia is only a relative contraindication – risks and benefits must be weighed, but there is evidence from animal models that giving antibiotics prior to the procedure can reduce infection [Carp et. al. Anesthesiology 76: 739, 1992]. Other relative contraindications include cardiac disease (avoid acute decreases in SVR), and abnormal coagulation studies (?)⁣
There is considerate controversy regarding the use of neuraxial anesthesia in patients with pre-existing neurologic disorders, such as multiple sclerosis. Baby Miller recommends avoiding it unless absolutely necessary.⁣

preaa Being an Anesthesiologist Assistant student at CWRU MSA Houston, we frequently participate in community service in and around the Houston area! This was one of my favorite community service events we as a group participated in! Project CURE works to send medical supplies overseas. On this day, I was in charge of assembling functional laryngoscopes kits with a complete set of both Mac and Miller blades, a pair of Magill forceps, handles, and batteries to assist in direct laryngoscopy intubations for surgical procedures! 
Community service is amazing, and I am so glad to be apart of a program that actively gives back to the medical profession!

A little morning Neuro Anesthesia dry set up coming at ya! 🧠 This set up takes a little bit longer than your typical set up due to the fact that the Alaris pump is needed in order to run infusions (Propofol, Remi/Sufentanyl, Precedex, Keppra, etc.) during the case along with 1/2 MAC gas! 
Students do not have access to the Pyxis until our Resident MD, Anesthesiologist, or CAA get to the room in the morning - this is why we call it a “dry” set up!

Many hospitals have an Anesthesia Pre-Op Clinic. Typically our patients with significant heart history, respiratory diseases, seizure/stroke history, blood disorders, or history of difficultly with anesthesia in the past come to the Anesthesia Clinic before their scheduled surgery. This allows the Anesthesia Care Team to adequately access the patient’s past medical history and thoroughly examine past medical records, without the “hurry up and wait” vibes on the day of surgery. 
Preoperative assessment allows identification of modifiable risk factors, optimization of the patients condition, explanation of the risks and formulating the best possible anesthetic plan to improve patient safety, optimize resource utilization and increase patient satisfaction. 
Anesthesia Pre Op Clinic allows us to expedite (and ease) the day of surgery process. No one wants anesthesia to be the one holding up surgery - although it is the butt of the jokes in the OR. Anesthesia has to make sure we cross our t’s and dot our i’s - we are the patients advocate while they’re under general anesthesia.

Preoperative assessment allows identification of modifiable risk factors, optimization of the patients condition, explanation of the risks and formulating the best possible anesthetic plan to improve patient safety, optimize resource utilization and increase patient satisfaction. Anesthesia Pre Op Clinic allows us to expedite (and ease) the day of surgery process. No one wants anesthesia to be the one holding up surgery - although it is the butt of the jokes in the OR. Right after our “Patient Experience” simulation lab on the first day, we are given a completely blank drug card and have to fill it out as our first test grade!

I had no idea what I was getting my future self into when taking this picture. Look how bright eyed and bushy tailed I was. 1st year is hard, but boy is it worth it in the end! 👩🏼‍⚕️ Hold on - It’s a wild ride!

AAs must always work under the supervision of a physician anesthesiologist, in a 4:1 ratio in the OR. The American Society of Anesthesiologists states that, “In no way is this a mark of inferiority but rather a realistic recognition that non-physician anesthetists must work within the relationship of a physician anesthesiologist to best ensure patient safety and optimal delivery of care.” This doesn’t mean the anesthesiologist is always in the OR with the AA – it simply means that the AA must collaborate with their physician to create an anesthetic plan for each patient and that the physician must be readily available should the AA need them.

preaa Commonly, in anesthesia, two different types of Neuromuscular Blocking Agents (NMBAs) are used -- Depolarizing and Non-Depolarizing. Succinylcholine falls into the Depolarizing NMBA category. ⁣

Succinylcholine, also known as Anectine, is composed of two acetylcholine (ACh) molecules linked together. It binds the post-synaptic neuromuscular junction (NMJ) motor end plate to cause depolarization at the acetylcholine receptor (AChR). Interestingly enough, endogenous ACh neurotransmitter molecules work the same way. Succinylcholine remains on the ACh Receptor, causing continued depolarization followed by a muscular relaxation/ Relaxation occurs as calcium is pushed back into the sarcoplasmic reticulum (SR).⁣

🔹A standard induction dose of sux will raise one’s potassium by 0.5 mEq.⁣
🔹Sux is relatively contraindicated in acute burn patients post 48hr up to two years due to upregulation of NMJ acetylcholine receptors⁣
🔹Succinylcholine can induce Malignant Hyperthermia. Important to inquire about family history.⁣
🔹The possibility of a phase II block should be suspected if succinylcholine is given as an infusion or if tachyphylaxis is noted with repeated doses. Redosing sux is a controversial practice.⁣

Commonly, in anesthesia, two different types of Neuromuscular Blocking Agents (NMBAs) are used -- Depolarizing and Non-Depolarizing. Succinylcholine falls into the Depolarizing NMBA category. ⁣ ⁣ Succinylcholine, also known as Anectine, is composed of two acetylcholine (ACh) molecules linked together. It binds the post-synaptic neuromuscular junction (NMJ) motor end plate to cause depolarization at the acetylcholine receptor (AChR). Interestingly enough, endogenous ACh neurotransmitter molecules work the same way. Succinylcholine remains on the ACh Receptor, causing continued depolarization followed by a muscular relaxation/ Relaxation occurs as calcium is pushed back into the sarcoplasmic reticulum (SR).⁣ ⁣ Important Mentions:⁣ 🔹A standard induction dose of sux will raise one’s potassium by 0.5 mEq.⁣ 🔹Sux is relatively contraindicated in acute burn patients post 48hr up to two years due to upregulation of NMJ acetylcholine receptors⁣ 🔹Succinylcholine can induce Malignant Hyperthermia. Important to inquire about family history.⁣ 🔹The possibility of a phase II block should be suspected if succinylcholine is given as an infusion or if tachyphylaxis is noted with repeated doses. Sorry IG, it’s been a minute! A few weeks ago, we had a lab to practice placing double-lumen endotracheal tubes (DLT), which are used for one-sided ventilation of either the right or left lung.  This procedure also involves the use of a fiber-optic bronchoscope (FOB) to confirm the placement of the tube in the correct bronchus. They can be tricky to maneuver at first so it’s important to learn how to get good at using a FOB so your attending doesn’t push you out of the way for taking too long. 🤷🏼‍♀️

Pulseless electrical activity, or PEA, is a common occurrence during arrest situations. In PEA, the monitor will show electrical activity in the heart, but the patient will have no palpable pulse. ⁣

✔️CAUSES ⁣
If you encounter a patient in PEA, it is important to know the Hs and Ts to help you find the cause of the problem! When you find the cause, it is sometimes (but not always) possible to reverse PEA and achieve ROSC (return of spontaneous circulation). ⁣
⁣
✔️SYMPTOMS⁣
It is important to not only take advantage of the monitoring we have available to us in medicine (EKG/ECG) but it is equally important to use your patient as a monitor as well. With PEA, you may see normal sinus rhythm, bradycardia, and possibly ventricular tachycardia. Despite having a rhythm on the monitor, the patient will not have a palpable pulse or blood pressure — the most important symptom of this condition is the disparity between what you see on the monitor and your assessment of the patient.⁣
⁣
✔️TREATMENT⁣
PEA is treated much like asystole. It is not a shockable rhythm because the electrical system in the heart is actually working properly. Shocking the patient is done to ‘reset’ the heart’s rhythm, but the problem in PEA isn’t in the conduction of electrical stimuli in the heart. CPR should be performed for at least two minutes, after which, both a rhythm and pulse check should be completed. If the rhythm is shockable, you would proceed with defibrillation. If the patient is still in PEA, you would continue with CPR, administer epinephrine and begin to consider possible causes. Epinephrine (1mg) can be given every 3 to 5 minutes.⁣

preaa Propofol (Diprivan) is a diisopropylphenol used in a number of settings. Most recently I’ve used it as a student in an intensive care setting for its heavy sedation effects to facilitate endotracheal tube tolerance and ventilator synchronization. 
Propofol is formulated as an oil-in-water emulsion for intravenous use, it is highly lipophilic and rapidly crosses the blood-brain barrier resulting in a rapid onset of action. Emergence from sedation is also rapid because of a fast redistribution into peripheral tissues and metabolic clearance.

Preservative: EDTA
Induction Dosing: 1.5-2.5mg/kg*
Infusion: 25-250mcg/kg/min*
IVP Push: 30-50mg*

preaa Blood 💉 Gas 💨 Coefficients: Uptake and Distribution of Volatile Inhalation Agents

Rate of uptake is dependent on the following;
1️⃣ Alveolar ventilation rate 
2️⃣ Partial pressure of gas (concentration effect) 
3️⃣ Breathing system

The blood-gas partition coefficient is the principal determinant of the rate in which the alveolar contraption (FA) increases towards a constant inspired concentration (Fi). The blood/gas partition coefficient describes how the gas will partition itself between the two phases after equilibrium has been reached. For example: Enflurane has a blood/gas partition coefficient of 1.7. Therefore, if the gas is in equilibrium the concentration in blood will be 1.7 times higher than the concentration in the alveoli. Thus, it makes sense that a gas with a higher blood gas coefficient will require higher uptake of gas into the blood and induction will be slower than Desflurane which has a blood/gas coefficient of 0.42.

Laryngoscope blades are typically used as the primary tool for the examination of the interior of the airway and for placement of an endotracheal tube. Choosing the correct size laryngoscope blade is critical to successful endotracheal intubation.

In 1943, Macintosh introduced the curved laryngoscope blade. The Mac blade is designed to lift the epiglottis indirectly and provide a view of the larynx by placing the tip of the blade in the vallecula. The Mac blade is typically less stimulating (less tachycardia and/or arrythmias), favored in patients with BIG TONGUES 👅👅, and lifts the epiglottis INDIRECTLY!

In 1946, Miller described a modification of the original laryngoscope, introducing a straight blade. Today, the Miller blade is one of the most popular laryngoscopes for pediatrics and difficult-to-intubate patients. Many practitioners prefer a straight blade laryngoscope on pediatric patient due to the unique upper airway anatomy of an infant or a small child. A straight blade is favorable in elevating the tongue 👅👅 so it is then removed from the field to facilitate optimal visualization of the infant larynx; however, there is a greater potential to damage the epiglottis with a straight blade, with the DIRECT elevation of the epiglottis itself rather and applying the indirect method.

Mac = Curved (easy to remember because the ‘C’ in Mac is curved)
Miller = Straight (easy to remember because the ‘l’ in Miller is straight)

At the time of this post there are 12 AA Programs in the country all of which offer a Masters Degree in Anesthesia. 📚💉 While each program will have their own unique personalities and have slight variances in curriculum and prerequisites, every program trains students to become highly skilled anesthesia providers who work within the Anesthesia Care Team model. preaa Big day for my piglets at @thestrengthfortress today!

@ziang4158 put everything into his performance today and went 8/9 in his first comp, gaining 32.5kg on his squat, 15kg on his deadlift and 7.5kg on his bench for a staggering 55kg total PB.
535 @ 90kg

@brisson_eh went 7/9 added 10kg to her squat, 2.5kg to her bench and 10kg to her deadlift despite losing her mind on her second bench and just coming short on her third deadlift.
285 @ 76

Ben Marriott went 8/9 equalling his best squat, adding 5kg on his bench making it into the lmao3plaet bench club and narrowly missing a 2.5kg deadlift PB.
532.5 @ 83

Subjunior Claudia went 8/9 also, losing the plot on bench a little after some troubles this cycle but still walking away with a 10kg squat pb and a 7.5kg deadlift PB.
